- Section 3.6 lists additional information that must be provided, but does not apply to every dangerous good shipped
- Any other additional documents that are required to accompany the shipment must be provided (i.e., those required by the “Packaging and Transport of Nuclear Substances Regulations”)
- Section 3.6.1 A shipping document must include one of the following certifications*:
- “I hereby declare that the contents of this consignment are fully and accurately described above by the proper shipping name, are properly classified and packaged, have dangerous goods safety marks properly affixed or displayed on them, and are in all respects in proper condition for transport according to the Transportation of Dangerous Goods Regulations.”
- the certification set out in section 172.204 of 49 CFR
- the certification set out in section 5;4.1.6 of the ICAO Technical Instructions; SOR/2017-137
- the certification set out in section 5.4.1.6 of the IMDG Code
- the certification set out in section 5.4.1.6 of the UN Recommendations
- The certification must be made by an individual who is the consignor or by an individual acting on behalf of the consignor and must set out that individual’s name
